Best strategy for handling an erroneous online application

What is the best strategy for processing an erroneous online application, where the youth’s name is the name of the father and the parent’s name is the name of the father as well, but with the mother’s contact information, age, and gender? I am thinking I should reach out to the family, “Do Not Accept” it and then have them reapply with some assistance. Is that the best strategy to avoid a big confusing mess, or since they already have assigned ids it will be best to let it go through and have council fix the names and everything?
