When I run a Recognition Report, I have Lion scouts under the Bear Den and a Wolf Scout under the Tiger Den. Any options for correcting this.

take a look at their active memberships - do they have unapproved memberships in the other dens?

No, both scouts are approved with a green check mark for their membership

I assume you are using the sort by den option?
Are the den names/numbers of the scouts showing up in the wrong section or completely missing?

OK, I’ve reproduced something similar where some den names don’t seem to be displayed at all. The scouts still seem to be grouped properly. For example, only one of our Bear den names is displayed. So, the report looks like all of the Bears are in one den. I’ve reported it to the developers. I’m hopeful the fix will catch your issue, too.

What device and browser are you using? Do you see anything different between the run and pdf buttons?

I am using a PC and Chrome. I have also tried to run the reports on my IPAD with the same result.
No difference between run and pdf buttons.

I had noticed some differences. iOS was worse than PC or PDF.
I do not know when this will be fixed. I suggest setting notifications on the change log forums. Thanks for reporting this.
