On the Webelos rank card template 645219, the names of the Den Leader (Melissa) and the Cubmaster (Lisa) are swapped. The Den Leader should be first and then the Cubmaster.
Also, the Scout name should be larger and lower, in order to be more centered on the card.
@LatnerG in SB+ I imagine both adults are assigned to the den - that is the issue.
