Permission Slips - Only First Scout is Filled Out

The permission slip generator (by checking permission slips required) only fills in the first scout, but then has blanks for the remaining number of Scouts. 2 Scouts? 1 filled in, 1 blank. 4 Scouts? 1 filled in, 3 blanks.

I have tried this with a couple of events. One of the events was a brand new event.
