SB+ shows I marked completed/approved but I didn't

I have a Scout who had a rank requirement show that it was marked completed by me on one date and approved on another date, but I did not key it.

For example, it shows that I keyed her specific requirement completed for Tenderfoot on 9/20 and approved on 11/13. However, the only things I did that day were mark her Scout rank complete on 9/20 and mark her Scout rank awarded on 11/13. I went back through the audit log and I don’t see anything showing me key anything for this scout for Tenderfoot. Could this be a system bug?

@KellyCiric- you may want to post the membership id of the scout in question

Thank you, it is 141737029. It was Tenderfoot #11 that showed that I marked completed and approved. I have since removed the date, since she has not done her BOR yet.
