yes, it shows me in all three (completed, approved, awarded). it shows me in at least three different dens and four different awards.

I entered the Tiger Bites AND Super Science. I know that I used Quick Entry for TB. I both marked them complete and approved at the same time.
Super Science was done over two different meetings. I used Quick Entry for the requirements we did most recently and marked everything as completed and approved. We had at least one scout who missed the first event so I ended up using the roster to go back and forth and make sure that all of the scouts who were at both meetings were correct.
@JoAnnaBarrow - why would a bear den leader enter a tiger requirement
he lead that meeting, he has a tiger, and is also the assistant cub master.

The simple answer is it looks like SB+ is overwritting Audit data - it is being looked at - you can watch the change log for updates,
