Unable to open detailed assessment sent by commissioner

Council unit representative was sent detailed assessment to be filled out. Unable to open form. 404 error. Last year able to open and fill out as a COR.

Additionally CUR showing up on not trained report even though all COR training done. No training linked to this new council position

With the CUR position being new, I can see how there may be some issues still. I will report this to the development team.
